Join us on a mission to advance medicine.As a Care Coordinator - RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum.
Our Fairview Southdale Cardiology Clinic supports a wide variety of patients with their cardiac needs. This includes preventative care, coronary artery disease, hypertension, heart failure, atrial fibrillation and more. We have a large group of providers that work closely with the RN's, clinical support staff, scheduling staff and more to provide excellent patient care. Southdale was rated #1 in the twin cities for cardiac care, and coronary intervention. The Care Coordinator - RN works to maximize the patient's health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator - RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and externalcustomers (providers, clinic staff, patients, and families).
Care Coordinator - RN Overview
Key responsibilities include but are not limited to:
- Coordinates care across the health care continuum within designated sub-specialty population(s)
- Serves as single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and services
- Completes pre-visit planning and post visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visit
- Facilitates proactive disease management and preventative health care using evidenced based practice
- Performs other duties as determined by manager and clinic needs
Minimum Qualifications:
- Current Minnesota RN license and minimum of ASN
- BLS certification
- 3+ years of clinical nursing experience
- Demonstrated experience in patient advocacy and education, coaching, and proactive disease management
- EMR and keyboarding/computer skills
